Please order twice as many of this form as you did of Copy A (LW2A), if the employer has to file a copy with the state (Copy 1) and also wants an Employer Record (Copy D). Order 3 times as many if the Employer has to file with the state and local Tax Department in addition to keeping an Employer Record copy.
